What Is Fatigue in Autoimmune Hepatitis (AIH)?

Fatigue is one of the most common symptoms experienced by people living with autoimmune hepatitis (AIH). Many patients describe it as one of the most difficult parts of managing the disease. Even when blood tests are stable and the disease is well controlled, individuals may still feel very tired.

Studies show that more than half of people with autoimmune hepatitis experience moderate to severe fatigue. Research, including work by Dr. Craig Lammert, has shown that fatigue affects many areas of life. This includes work, relationships, and emotional well-being. Many patients describe it as one of the most frustrating and limiting parts of their condition.

The impact of fatigue in autoimmune hepatitis (AIH) can be broad. It can make it harder to:

  • Keep up with work or school.
  • Stay socially connected.
  • Maintain a sense of well-being.

This is why it is important to recognize fatigue as a real and meaningful part of living with AIH. It is not something to ignore or simply “push through.”

Fatigue in AIH Is More Than Just Sleepiness

When people think of fatigue, they often imagine simple tiredness or the need for sleep. However, fatigue in autoimmune hepatitis is more complex and can affect both physical energy and mental clarity.

Patients frequently describe this fatigue as a heavy, weighed-down feeling, like the body is drained of energy. It can make small tasks seem overwhelming. Many individuals also experience “brain fog,” which can affect focus, memory, and clear thinking. Others report low motivation and reduced physical stamina, even for activities they once enjoyed.

Unlike typical tiredness, this type of fatigue is not always relieved by rest. You may sleep through the night and still wake up feeling exhausted.

How to Track Fatigue in Autoimmune Hepatitis (AIH)

A helpful first step is to begin tracking your fatigue. Tools like the Fatigue Severity Scale (FSS) or a simple 0–10 rating can be used. Some digital health apps, such as Bearable, Folia Health, and CareClinic, can help you better understand your symptoms over time.

When tracking your fatigue, consider recording more than just how tired you feel. The more detail you provide, the more useful the information will be for both you and your care team.

What to Track

  • Severity of fatigue
    • Use a scale (for example, 0 = no fatigue, 10 = worst imaginable fatigue).
  • Impact on daily life
    • How does fatigue affect your ability to function? Does it interfere with work, school, exercise, or social activities? Does it interfere with simple tasks like cooking or getting dressed? Be sure to note the specific activities that are affected.
  • Timing of fatigue
    • When does the fatigue occur? Is it worse in the morning, afternoon, evening, or all day?
  • Associated symptoms
    • Do any symptoms occur with your fatigue? Examples include: brain fog, sleep disturbances, pain, mood changes, or headaches.
  • Possible triggers
    • Does anything seem to worsen your fatigue? Does poor sleep, stress, medications, diet changes, illness, or physical exertion worsen the fatigue?

Fatigue in autoimmune hepatitis (AIH) is common, real, and often challenging, but it can be understood and managed. Paying attention to your symptoms and tracking them over time is an important first step toward improving your quality of life. Be sure to share your results with your health care team.
