The Transition
The transition from pediatric health care to adult care can be overwhelming, especially for parents. Give your child the tools to become independent by starting to think about the transition when your child is 12-14 years old. Over time, they can become prepared to assume responsibility for their own care and manage their symptoms, doctor visits, lab results, and insurance.
